RN Case Manager
The RN Case Manager provides high quality patient care in both the home health and hospice and palliative care setting. This position is an integral member of our hospice care program, and is responsible for coordinating and providing quality end-of-life care to patients in their homes or residential facilities. The role involves comprehensive patient assessments, care planning, coordination of services, and compassionate support for patients and their families. The RN Case Manager educates patients and families about hospice services, disease progression, medication management, and advance care planning along with providing skilled nursing interventions to manage symptoms and promote comfort for patients receiving hospice care.
Responsibilities:
- Develop and implement nursing care plans.
- Facilitate coordination of physical, clinical, and psychosocial care among the interdisciplinary healthcare team and patient/family.
- Use technology to assess the health status and physical needs of the patient.
- Assess patients’ knowledge and risk for complications.
- Set and reach realistic goals for patients.
- Provide education to patients and families regarding appropriate home care.
- Assist patients in arranging for needed home health services.
- Provide education to patients about their level of physical dependence on others for activities of daily living, daily routines, and when to seek out others for help.
- Provide education to caregivers about supportive care measures to incorporate into the home environment.
- Participation in weekend on-call rotation.
Education:
- Minimum Associates Degree in Nursing; BSN preferred.
- Current Professional Registered Nurse license issued by the Illinois Board of Nursing.
- CHPN preferred.
Experience:
- One year of Medical/Surgical, Oncology, or Hospice Nursing experience.
- CPR Certification required. Knowledge of LCD’s required.
