Northern Illinois Hospice Foundation’s Grateful Families Program lets you show your appreciation for the care your loved one and family received from our outstanding team.

Share Your Story
In your own words, share your story with us and help others understand how Northern Illinois Hospice made a difference in you and your loved one’s life.
Recognize a Caregiver
You may have encountered a physician, nurse, certified nursing assistant, chaplain, social worker, music or massage therapist, volunteer or other team member who brought you an extra measure of comfort and reassurance in your time of need. The Grateful Families Program gives you the opportunity to recognize this special person(s) and make a gift in their honor.
When making your donation, please select “Grateful Family” in the drop-down menu, and if you wish, use the “Any Additional Information About Your Gift?” section to recognize more than one team member and/or share your story.
