We are currently seeking a full-time RN Case Manager / Visit Nurse to provide high quality patient care in both the home health and hospice and palliative care setting. This position will train as a Visit Nurse and transition into a RN Case Manager role as the agency’s needs arise.

Responsibilities:

• Develop and implement nursing care plans.
• Facilitate coordination of physical, clinical, and psychosocial care among the interdisciplinary healthcare team and patient/family.
• Use technology to assess the health status and physical needs of the patient.
• Assess patients’ knowledge and risk for complications.
• Set and reach realistic goals for patients.
• Provide education to patients and families regarding appropriate home care.
• Assist patients in arranging for needed home health services.
• Provide education to patients about their level of physical dependence on others for activities of daily living, daily routines, and when to seek out others for help.
• Provide education to caregivers about supportive care measures to incorporate into the home environment.

Experience:  minimum of 2 years healthcare experience and have a passion for providing exceptional patient care to patients.
