Job Summary: Under the direction of the Team Leader, provide spiritual and emotional support when needed to the patient/family and to members of the Northern Illinois Hospice interdisciplinary group (IDG). Provides assistance at the time of the patient’s death and support of the family as requested.
Essential Duties and Responsibilities:
· Adheres to organizational policies and procedures.
· Follows infection control policies and procedures; utilizing PPE as required.
· Follows building protocol.
· Attends mandatory education in-services and meetings.
· Maintains confidentiality according to HIPAA requirements of patient and family regarding disease/diagnosis, financial, and other information obtained during employment.
· Abiding by agency proprietary confidential information policy.
· Demonstrate the Model visit on each visit encounter.
· Follows safety education including physical ergonomics.
· Maintains personnel and professional boundaries.
· Provides accurate, timely documentation, pertinent, and complete data in the electronic medical record, according to documentation guidelines.
· Provides direct spiritual care, support and or counsel to hospice patients and families/caregivers om keeping with patients and families beliefs.
· Works with interdisciplinary team members, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
· Assures spiritual assessment of patients/families in the hospice program within 5 days of admission.
· Participates in the interdisciplinary group (IDG) by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated.
· Assist with the development of the individualized plan of care to meet the spiritual needs.
· Coordinates the assessment and delivery of grief counseling needs and services to ensure timely and appropriate services are provided to patients, family/caregivers.
· Provide bereavement follow up as assigned.
· Serves as liaison with local clergy and area churches to support patient and/or family requests and offer education through congregations about hospice care.
· Contacts clergy or appropriate representatives of patient/families as indicated.
· Provides for funeral or memorial services for hospice patients as requested.
· Works with the NIH Memorial Service Planning Committee to plan and execute the annual ceremonies.
· Available to patients and families in the “On-Call” rotation for spiritual counseling/grief issues and assists with psych-social, crisis and/or death visits.
· Assist with the staff bereavement program and monthly newsletters as directed by the Bereavement Coordinator or Director of Clinical Services.
· Provides bereavement information and referral services to callers for the community.
· Maintains current knowledge of Medicare/Medicaid benefits, private insurance, supportive care, continuous care, and in-patient requirements.
· Remains current on local, state, and federal laws and regulations governing the operations of hospices.
· Adheres to professional standards of practice within their discipline.
· Provides and promotes superior customer service, exhibits good communication skills with patients, families, team members and professional community. Presentations as assigned.
· Participates in quality improvement activities to ensure highest levels of service.
· Other duties assigned.
Education: Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) or appropriate certification in hospital or pastoral ministry.
Training and Licensure: Certificate or degree in pastoral ministry.
Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgements. Ability to respond appropriately to crisis in community or facility setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
Experience: Minimum 2 years of experience as a chaplain/spiritual counselor, preferred. Active patient contact within past three years, preferred. Experience working with death and dying, individuals/ family/ caregiver and experience with clinical grief and/or pastoral counseling.
Other: Local residence with ability to respond to on-call within 30 minutes. Meets agency requirements for criminal background check.
